HUISARTSENPRAKTIJK HORTUS IS OPEN VOOR NIEUWE PATIENTEN!
Wij kunnen zorg leveren aan patiënten woonachtig in de postcodes: 2311, 2312, 2313, 2321 of 2332 (nb dit geldt voor patiënten die op dit moment niet staan ingeschreven bij een andere huisartspraktijk in Leiden). Buiten dit gebied nemen wij helaas geen nieuwe patiënten aan. Dit heeft te maken met de ‘aanrij-/ aanfiets-tijd’ indien er een spoedvisite gereden moet worden. Wij hopen op uw begrip hiervoor.
Let op: Wij nemen dus geen verzoeken in behandeling voor het wisselen van huisarts binnen Leiden.
Vanwege de privacy-wetgeving is het niet mogelijk om uw ingevulde inschrijfformulier per mail naar ons op te sturen. Wij willen u vragen dit ingevulde formulier bij de assistente af te geven, tezamen met uw paspoort of ID ter identificatie.
We can provide care to patients living in the zip codes: 2311, 2312, 2313, 2321 or 2332 (and for patients who are not currently registered with a GP in Leiden). Unfortunately, we do not accept new patients outside of this area due to the leadtime it takes in case we need to make an emergency visit. We appreciate your understanding.
Please note: We do not process requests for switching GPs within Leiden.
Due to privacy legislation, it is not possible to send us your completed registration form by e-mail. We would like to ask you to hand over this completed form to the doctor’s assistant, together with a valid ID (passport / ID card) to identify yourself.
