PATIENTENSTOP / PATIENT-STOP per 1 mei 2025

Helaas is Huisartsenpraktijk Hortus per 1 mei 2025 (tijdelijk) gesloten voor nieuwe inschrijvingen. Om toereikende zorg te kunnen blijven leveren met ons team zijn we gebonden aan een maximum totaal aantal mogelijke patiënten in onze praktijk. We werken met een wachtlijst waarop u zich vrijblijvend mag aanmelden bij de balie. Zodra er plek vrijkomt zullen we mensen van de wachtlijst gaan benaderen of zij zich willen inschrijven als nieuwe patiënt.

Een plek op de wachtlijst is vrijblijvend voor beide partijen. Wij kunnen niet verantwoordelijk worden gehouden voor het verlenen van (acute) zorg voor mensen op de wachtlijst.

 

Unfortunately, Huisartsenpraktijk Hortus will be (temporarily) closed for new registrations as of May 1, 2025. In order to continue to provide adequate care with our team, we are limited to a maximum total number of patients in our practice. We work with a waiting list for which you can register at the counter without obligation. As soon as space becomes available, we will approach people on the waiting list to ask if they would still like to register as a new patient.

A place on the waiting list is without obligation for both parties. We cannot be held responsible for providing (acute) care for people on the waiting list.
